The Inside-Out Problem

Why your best people still can't see the misalignment
Here's something most leadership teams don't want to hear: the seniority of the person looking at a problem has almost nothing to do with whether they can actually see it.
Misalignment in large organisations doesn't live inside any single team. It lives in the gaps between workstreams, between functions, between what Technology thinks "done" means and what the Product team thinks "ready" means. And here's the catch: you can only see those gaps from a position that spans the whole ecosystem.
No one inside the system occupies that position. Not the CEO. Not the Programme Director. Not even the steering committee, however cross-functional it claims to be. Every internal actor, regardless of title, is embedded somewhere in the structure, carrying the perspective, incentives, and blind spots that come with that position.
You cannot simultaneously be inside the machine and observe the machine from outside it.
This is why internal reviews so often miss the thing that later turns out to be obvious in hindsight. It's not a competence problem. It's a vantage point problem. And vantage point problems don't get solved by promoting someone, hiring someone smarter, or adding another layer of governance.
They get solved by changing where the observer is standing.
That's structurally only possible from outside.
